Department of Health and Family Welfare has issued Office Memorandum No: S.11011/23/2009-CGHS D.II/Hospital Cell/Part IX dated 30.04.2012 for empaneling certain private hospitals for treatment under CGHS in Lucknow.  The following list provides the details of hospitals category wise
